- Circle purchase price (Limit one model per product category).
- Ensure that the envelope is postmarked within 30 days of purchase.
Only Artisan brand products purchased from a participating retailer between April 1st and September 30th, 2020 are eligible. Offer requires this completed mail-in form, and a legible copy of store receipt with the purchase prices circled. Incomplete submissions will be returned to the sender for re-submission. Claim must be postmarked no later than October 31st, 2020. Re-submissions must be postmarked by November 15th, 2020. Only handwritten claims will be processed. Allow up to 8 weeks for processing. Submitted materials will not be returned. Not responsible for lost, late, or misdirected mail.
